2001 Summer Public Health Research Videoconference on Minority Health

Site Agreement

This document sets forth the terms and conditions for institutional participation in the Summer Public Health Research Videoconference on Minority Health


The University of North Carolina, School of Public Health agrees to:

(a) Distribute via satellite transmission, five 3-hour videoconference sessions of the 2000 Summer Public Health Research Institute on Minority Health on June 18-22, 2001; and
(b) Make available selected promotional and instructional materials that Licensed Sites may duplicate for use in connection with the Videoconference.

With the agreement, Licensed Sites have the right to:

(a) Access the Videoconference via satellite transmission;
(b) Make and retain one (1) videotape recording of the Videoconference; and
(c) Transmit the Videoconference on a closed-circuit system located at the facilities of the Licensed Site.

The Licensed Site hereby agrees to designate an on-site facilitator who will be responsible for the following:

(a) Provide a room of adequate size to support the expected audience at the site;
(b) Prepare the facility including arranging for sufficient audio/visual monitors and equipment, seating, and (optionally) refreshments;
(c) Greet attendees and providing an introduction of the day's event(s);
(d) Provide information and assistance to local attendees as the need arises;
(e) Coordinate attendee registration, if any, for your site; and
(f) Provide an accurate count of the number of participants during each segment of the broadcast.

The Licensed Site agrees to the following conditions:

(a) The copyright in Conference recordings belongs to the presenters or to the University of North Carolina at Chapel Hill School of Public Health; these materials shall not be sold or rented, nor shall they be disseminated beyond the licensed facility, by any means, without written approval of the UNC School of Public Health.
(b) Recordings of the Conference shall not be edited or altered;
(c) The Licensed Site shall give proper attribution to the UNC School of Public Health when transmitting the Conference on any system not located within the Site's facilities; and
(d) The UNC School of Public Health shall not be liable for failure to deliver the Conference broadcast due to circumstances beyond their control.
